But reports do not show that the National Highway Traffic Safety Administration (NHTSA) opened a formal investigation into that crash. NHTSA gathered information; the separate federal investigation reported at the time concerned electric door handles on other Tesla models.
What happened in the Piedmont Cybertruck crash?
Four people were in the Cybertruck when it left the road shortly after 3 a.m., struck a retaining wall and caught fire, according to the Associated Press. Three occupants died. The fourth, Jordan Miller, survived after a person from another car pulled him from the wreckage.

The California Highway Patrol (CHP) attributed the crash in a preliminary report to drug use and excessive speed. AP quoted the agency: “Officers determined that a combination of driving under the influence of drugs and unsafe speed were the causes.” That was a preliminary state finding reported while the investigation was ongoing—not a final determination or a federal finding.
Did NHTSA open an investigation into this crash?
Not according to the reporting available. The Guardian reported that NHTSA was gathering information from Tesla and law enforcement after the Piedmont crash, but had not opened a formal investigation into it. The Guardian’s March 2026 report and KTVU’s coverage describe a separate federal inquiry concerning electric door handles on Tesla Model 3 and Model Y vehicles. Those reports do not establish that the Piedmont Cybertruck crash was part of that inquiry.

The distinction matters: information-gathering by NHTSA is not the same as a formal safety-defect investigation, and a separate investigation into other vehicles is not an investigation into this collision.
How a crash inquiry differs from a vehicle-defect investigation

Tesla’s Cybertruck owner’s manual explains that NHTSA may investigate similar complaints and can order a recall and remedy campaign if it finds a safety defect in a group of vehicles. It also says the agency cannot resolve an individual dispute between an owner and Tesla.

What reports about exits and rescue do—and do not—show
Separate reporting has raised concerns about people or rescuers having difficulty opening or breaking vehicle exits after crashes. The Guardian’s March 2026 coverage says Cybertruck windows use high-density laminated glass that is harder to break than regular car windows and describes a survivor rescued after bystanders worked to break a window. Such accounts provide safety context, but they do not establish that an exit or glass defect caused the Piedmont crash or that a particular escape tool will work on Cybertruck glazing.
KTVU described manual door-release locations as a front-door latch near the window switches and a rear release under a plastic well in the map pocket. Because instructions may depend on vehicle configuration and manual revisions, owners should consult the current manual for their Cybertruck rather than rely on a news description.

How to report a suspected Cybertruck safety defect
- Check the current Cybertruck owner’s manual and document the suspected problem, including when and how it occurred.
- Notify Tesla and NHTSA if you believe the defect could cause a crash, injury or death. Tesla’s manual says to inform both.
- Understand that a complaint may contribute to NHTSA’s review of similar reports, but does not itself establish a defect or guarantee an investigation or recall.
