Regular and premium gasoline differ mainly in octane: premium resists engine knock better, but it is not automatically more powerful or more efficient. Use the grade specified in your owner’s manual. If premium is only recommended, any benefit depends on the vehicle—and may not make up for its higher price.
What is the difference between regular and premium fuel?
In the United States, the main difference is the fuel’s octane rating. A higher rating means greater resistance to knock, an abnormal combustion event. It does not, by itself, mean the fuel contains more energy per gallon or will improve every car’s performance or fuel economy.
U.S. pump labels use the anti-knock index, or AKI, shown as “(R+M)/2.” It is the average of two laboratory ratings: research octane number (RON) and motor octane number (MON). Because some markets label fuel using RON alone, their pump numbers are not directly comparable with U.S. AKI numbers. See the U.S. DRIVE technical report hosted by the Department of Energy.
“Regular,” “midgrade” and “premium” are market-grade names, not a universal ranking of fuel quality. In a 2022 summary, DOE used these U.S. AKI ranges: regular, 85 to below 88; midgrade, 88 through 90; and premium, above 90. Grades offered and local requirements can vary, including with altitude. These ranges are from DOE’s 2022 summary of fuel grades and historical prices.

What octane does inside the engine
In a spark-ignition engine, the spark plug starts a flame that travels through the fuel-air mixture. Knock happens when some of the unburned mixture autoignites ahead of that flame. That abnormal combustion can create a rapid rise in pressure; DOE’s technical report warns that, if left unchecked, it can degrade or damage an engine.
Higher compression can improve engine efficiency, but it also raises the likelihood of knock. Turbocharging and other high-output, downsized designs can make an engine more knock-limited in some conditions. Manufacturers may specify higher octane so an engine and its controls can operate as designed. The useful effect depends on the engine, not simply on the fuel having a larger number. DOE discusses this relationship in its overview of engine design and octane.

Does your car require premium or just recommend it?
These instructions are not interchangeable. “Required” means you should use the grade specified by the manufacturer. DOE states: “Vehicles that require premium gasoline should always be fueled with premium gasoline as stated in the owner’s manual.” Check the manual and the fuel label inside the fuel door; DOE’s consumer guidance on premium fuel explains the distinction.
“Recommended” but not required means the car can use a lower grade, but premium may help it achieve some of its rated performance or fuel economy in particular conditions. There is no universal horsepower or miles-per-gallon increase that applies to every car with this recommendation. Consult the manufacturer’s guidance for your specific model.

If the manual specifies regular, premium is unlikely to provide a meaningful benefit. It will not turn an engine designed for regular into one designed around premium. If the manual and fuel-door label appear to conflict, or the vehicle has been modified, confirm the correct grade with the manufacturer or a qualified technician.
Which fuel should you choose?

DOE reported that, among model-year 2023 vehicle configurations, 26.7% required premium and 26.3% recommended it without requiring it. These are configuration-level figures—not the share of vehicles currently on U.S. roads. DOE counts different make, model, engine and transmission combinations separately. The figures and definition appear in DOE’s July 2024 summary.

Is premium worth the extra cost?
Compare the local price difference with a benefit documented for your vehicle. As historical context, DOE reported a 68-cent-per-gallon average U.S. price difference between premium and regular in 2021 dollars, based on EIA data; the reported difference peaked at 70 cents per gallon in 2020. These are historical national figures, not current prices at a particular station. The figures are in DOE’s December 2022 summary.
For a car where premium is recommended but not required, DOE says the fuel-economy benefit usually does not offset the added cost. A particular model may still offer a performance or economy benefit, so follow the vehicle-specific guidance and decide whether that benefit is worth paying for.

Octane is not the same as energy content
Octane describes resistance to knock, not how much energy a gallon contains. Ethanol makes the distinction clear: it has a higher octane number than gasoline but lower energy density, and its effect on fuel economy varies with the blend. DOE’s Alternative Fuels Data Center explains the relationship. A higher-octane label alone therefore cannot establish that a fuel will deliver more miles per gallon.
