Nelson Thomas Blames Himself After Crash Led to Foot Amputation (Exclusive)

Nelson Thomas’s March 2023 crash in Austin, Texas, became a long medical recovery involving six surgeries, extensive hardware and, ultimately, the loss of his right leg below the knee. In a February 2024 interview with PEOPLE, the reality-TV personality said he blamed himself for the wreck and for the decisions surrounding it.

Thomas was known for MTV’s Are You the One? and The Challenge. He was 35 when the interview was published. At that point, he was preparing for an amputation scheduled for March 5, 2024—the first anniversary of the crash. Later reports confirmed that the operation took place as planned.

What happened in the Austin crash?

Thomas said he hit a pillar in Austin in March 2023 and later woke up to find his vehicle engulfed in flames. Minnesota Vikings player K.J. Osborn and three other people helped pull him from the burning car.

The crash left Thomas with severe injuries to his right foot and ankle. His treatment included six surgeries, three plates and 22 screws. He also tried traditional physical therapy, electrical stimulation and stem-cell treatment during the recovery process.

Thomas was charged by the Austin Police Department with misdemeanor driving while intoxicated on September 28, 2023, in connection with the crash. A charge is an allegation, not a conviction, and should not be described as proof of a criminal conviction without a separate court record.

Why Thomas said he blamed himself

Thomas told PEOPLE that media coverage of the DWI charge and harsh comments on social media affected him while he was recovering. His own description was stark: “I blame myself for everything.” He said he accepted responsibility for what he called the biggest mistake of his life, while also recognizing that he needed therapy to deal with the emotional consequences.

The distinction matters in a crash story. Physical injuries can be visible and documented, while guilt, shame and online criticism can complicate rehabilitation. Thomas described both as part of his recovery rather than treating the accident only as a medical event.

Why doctors ultimately recommended amputation

Thomas initially began walking again after roughly three months, sooner than doctors expected. The improvement did not last. Doctors later identified a nonunion, meaning the injured bone was not healing properly.

He was given alternatives to amputation, including continued efforts to salvage the limb and an ankle or tibio-talo-calcaneal (TTC) fusion. Thomas said the fusion could leave him with little or no ankle movement, continuing pain and the possibility of more hospital stays and procedures.

Against that backdrop, he chose amputation. The February 2024 article referred to the procedure as a foot amputation, but subsequent reporting gave the more precise description: a below-knee amputation of his right leg.

Date What was reported
March 2023 Thomas was involved in a near-fatal crash in Austin and was rescued from the burning vehicle.
September 28, 2023 He was charged with misdemeanor DWI in connection with the crash.
February 24, 2024 PEOPLE reported that he had decided on a right-foot amputation, with surgery scheduled for March 5.
March 5, 2024 Later reports confirmed that Thomas underwent a below-knee amputation of his right leg.

The later update on Thomas’s recovery

In a later PEOPLE interview published in August 2026, Thomas said he was returning to The Challenge: Cutthroat after adapting to specialized prosthetics. The report described him competing with a prosthetic, a first for the franchise.

Thomas again acknowledged that he drank and drove and said he had taken responsibility for that decision. His return to competition illustrates the practical side of recovery: rehabilitation did not end with surgery, but continued through prosthetic fitting, training and adapting to a new way of moving.

What drivers can take from the crash

The case is also a reminder that impaired driving can create consequences far beyond a citation or damaged vehicle. A crash can affect passengers, other road users, rescuers and the driver’s long-term health. Thomas’s account also shows why the legal status of a case should be reported carefully: the September 2023 DWI filing was a charge, while the permanent medical consequences were independently described through his interviews and later reporting.
