On the morning of Wednesday 26 October 2022, a cement truck and a Kia Carnival collided head-on on Goomalling-Toodyay Road at Wongamine, in the Wheatbelt about 100km north-east of Perth. The Carnival’s driver, Hannah Louise Fraser, 30, died at the scene. Six children were in the vehicle and were injured. Police said the circumstances were under investigation, and no cause, charge or coronial finding had been published when this article was prepared.
What happened
Emergency services were called shortly after 6am. ABC News reported the call at about 6:15am, while the WA Police appeal gave the time of the collision as about 6:20am. The two times describe different moments and are not in conflict, so “shortly after 6am” is the safest way to describe the timing.
According to the WA Police appeal, as carried by Mirage News, the cement truck was travelling east and the Kia Carnival was travelling west when the two vehicles collided head-on. The crash scene is on a rural stretch between Toodyay and Goomalling.

Hannah Louise Fraser, 30, was the driver of the Kia Carnival. Emergency crews tried to save her, but she died at the scene. Media reports said her family was understood to be from Maddington. A Find a Grave memorial in her name exists, but it is user-generated and is not an official source.

The six children
Six children were in the Carnival. Their ages were given only as believed to be between one and 14, and no individual ages have been published in the reports reviewed. Four were flown to hospital by rescue helicopter, and two were taken by ambulance to Perth Children’s Hospital.
A hospital spokesperson told ABC News that, at the time, two of the children were in critical condition and four were stable. No later hospital update was found, so those conditions should be read as the status on the day, not the children’s current condition.

The truck driver
The 58-year-old driver of the cement truck was not seriously injured. He was taken to Northam Hospital as a precaution.
Where the crash happened
Goomalling-Toodyay Road runs through farmland and bushland in the northern Wheatbelt. The collision occurred at Wongamine, between Toodyay and Goomalling. The Major Crash Investigation Section asked witnesses to come forward. Police said the circumstances were under investigation. No cause has been published, and no charges or coronial inquest had been reported in the sources reviewed. Any statement about why the collision happened would be speculation, so this article does not offer one.
A later report about the same road
A syndicated report published later said that another fatal crash occurred within about 100m of the same spot roughly ten days afterwards, and described it as the third fatality in a month on Wheatbelt roads. This claim comes from that report, and it has not been independently verified in the sources reviewed.
