The opening summary listed 22 reports, including 17 crashes; it listed no injuries or fatalities among those initial reports. NHTSA later identified nine additional similar reports. Those counts describe reports under review—not a final finding that a defect caused every incident or a crash rate that can be compared with human drivers.
What was NHTSA investigating?
NHTSA’s Office of Defects Investigation (ODI) opened Preliminary Evaluation PE24016 on May 13, 2024. Its opening resume said ODI had received reports involving Waymo fifth-generation ADS vehicles that either collided while they were the sole vehicle involved or displayed behavior that might violate traffic-safety laws.
The reports described more than one kind of event: collisions with gates, chains, parked vehicles and other stationary or semi-stationary objects, as well as apparent violations of traffic-control devices or rules. The concern was therefore broader than a single pattern of striking parked cars.
What did the reported numbers show?
The opening resume’s summary listed 22 incident reports, including 17 crashes, with zero injury incidents and zero fatality incidents in that initial group. These are raw case counts from the opening summary, not an exposure-adjusted crash rate or a comparison of Waymo vehicles with human drivers.

In a May 23, 2024, information request to Waymo, ODI said it had identified nine additional similar reports since opening the evaluation. That later figure has a different date and scope from the initial 22-report summary, so it should not be added to the opening count without qualification.
Why did the agency raise concern?
ODI described examples including collisions with parked vehicles or barriers, entering an opposing lane with oncoming traffic nearby, and driving into construction zones. The agency noted that some events occurred near pedestrians or other road users and warned of possible crash, property-damage and injury risks. In its May 23 information request, ODI wrote: “ODI is concerned that ADS-equipped vehicles exhibiting such unexpected driving behaviors may increase the risk of crash, property damage, and injury.” This was the agency’s stated concern while requesting information, not a final causal determination.

The Associated Press reported at the time that Waymo said it would work with NHTSA and cited its safety record. That was the company’s position as reported in 2024, not an independent performance assessment.
What software remedies appear in later recall records?
NHTSA’s later recall records document software remedies for particular conditions. They provide relevant context, but do not show that every PE24016 report shared the same cause or establish the evaluation’s final disposition.

What a preliminary evaluation does—and does not—mean
NHTSA’s Standing General Order on crash reporting requires manufacturers and operators to report certain crashes involving Level 2 advanced driver-assistance systems or Level 3–5 automated driving systems when the system was engaged during or immediately before a crash. The agency uses reports to identify possible safety issues; they may lead to information requests or a defect investigation. A reported crash or an investigation by itself does not mean NHTSA has determined a defect exists.
Likewise, the opening incident counts do not establish how often such events occur relative to miles driven or how Waymo compares with human drivers. The cited investigation and recall records provide no exposure-adjusted comparison or generalizable comparative-safety statistic.

Is the federal investigation still open?
The final disposition of PE24016 is not established by the cited records here. NHTSA’s investigation docket is the place to confirm whether the evaluation remains active or has closed; the recall notices alone do not answer that question.
