Stop driving and arrange a qualified diesel diagnosis. A Ford diesel that loses power, runs roughly, stalls, cranks without starting, or logs a fuel-pressure code may have a failed pump, but it may also have a restriction, wiring fault, control-valve problem, injector issue, or contaminated fuel. Continuing to run or repeatedly crank it can turn a diagnosis into expensive fuel-system or engine damage.
What to do immediately
If the engine is still running
- Move to a safe location without accelerating hard or trying to “clear” the fault. Loss of rail pressure can leave you without power or cause a stall in traffic.
- Switch the engine off if it is knocking, smoking unusually, surging, losing power, or showing a warning message. Do not continue driving to reach a preferred shop.
- Call for a tow and tell the technician that a diesel-fuel-system failure is suspected. If you may have added gasoline, DEF, water-contaminated fuel, or another wrong fluid, say so before anyone restarts the truck.
If it has stalled or will not start
Avoid repeated cranking. Each attempt can move contaminated fuel or abrasive debris farther through the system and can discharge the battery. Leave the vehicle where it is safe, arrange transport, and let the repair facility decide whether a controlled diagnostic start is appropriate.

Record the information the shop needs
- Model year, model, engine displacement, and whether it is a pickup, cab-and-chassis, or another configuration.
- Mileage and the exact sequence of events: hard starting, long crank, rough running, smoke, knock, loss of power, stall, or no-start.
- Whether the engine cranks normally, starts briefly, or never fires.
- Warning lamps, messages, and every diagnostic trouble code, including freeze-frame data if available.
- Where and when it was last fueled, whether the tank was recently filled, and whether a fuel filter or other fuel-system work was just performed.
These details matter because Ford’s repair procedures differ by engine, model year, fuel-system layout, and cause. A single symptom does not identify the failed component.
Why “the fuel pump failed” is not a diagnosis
Ford diesels have a low-pressure supply side and a high-pressure injection side, with sensors, valves, wiring, filters, and controllers that can all create similar symptoms. A restricted filter can starve a pump. An electrical short can disable a circuit while the pump itself remains serviceable. A high-pressure pump can be mechanically damaged by contamination, while a low-pressure fault may prevent it from receiving enough fuel.

How to interpret codes without guessing
Ford’s Diesel System Operation Summary, revised August 25, 2023, lists separate monitor families for different parts of the fuel system. The exact calibration and application for your VIN control the diagnosis.

Ask the repairer which tests turned the code into a component diagnosis. A scan result alone is not enough authorization to replace a pump. The shop should explain pressure measurements, electrical checks, filter inspection, and any evidence of metal or other debris.
Ford examples that show why model matching matters
Specific 2008 6.4L F-Super Duty electrical fault
A Ford bulletin issued in 2007 for certain 2008 6.4L F-Super Duty vehicles describes no-start or lack-of-power complaints with P0003, P0004, or P0091 and, in some cases, a blown fuse 74. The documented cause is a short to ground at the pump-cover gasket circuit. In that diagnosed situation, the procedure says the pump itself does not need replacement. This is an example of an electrical or gasket-related fault mimicking a pump failure; it is not a diagnosis for every 6.4L truck.

2011–2012 6.7L DEF-contamination bulletin
Ford TSB 11-10-10, published October 10, 2011, covers a narrow group of 2011–2012 F-Super Duty 6.7L cases involving P1291 or P1292. The bulletin links the condition to an internally shorted injector that may be associated with DEF-contaminated fuel or fuel gelling. For its specified DEF pathway, the technician checks whether the filter or filter bowl turns white after two hours of drying. If it does, the bulletin identifies DEF contamination as non-warrantable and calls for replacement of the complete high-pressure fuel system and diesel fuel conditioning module, followed by flushing the base fuel system.
That is a vehicle- and condition-specific procedure, not a blanket instruction for every Ford diesel, every P1291/P1292 code, or every contaminated-fuel event. Current Ford service information for the exact VIN takes precedence over an old bulletin.

Possible sources include accidental misfueling, water entering a storage or station tank, aged diesel, and microbial growth when fuel sits unused for an extended period. Contaminant-related debris can pass through filtration and damage injectors. The eventual repair depends on what entered the tank, how much entered, whether the engine ran, and what inspection finds.
If contamination is possible
- Do not restart the engine or keep cranking after the vehicle is safely stopped.
- Tell the tow operator and repair facility exactly what may have been added or encountered, including gasoline, DEF, water, additives, or a recently serviced fuel system.
- Ask the shop to retain fuel and filter samples when appropriate and to document the inspection.
- Require a written explanation of which parts the applicable Ford procedure replaces, which parts it flushes, and whether metal debris was found.
Do not assume draining the tank and installing a filter is sufficient. In some cases the contamination remains limited; in others it reaches pumps, injectors, rails, lines, and the fuel-conditioning assembly.

How a qualified technician should diagnose the fault
- Confirm the complaint. The technician records crank speed, starting behavior, smoke, noise, warning messages, and operating conditions.
- Scan all relevant modules. Codes are saved before clearing, with freeze-frame information and commanded-versus-actual pressure data.
- Check fuel quality and restriction. The filter, water separator, fuel sample, lines, and evidence of air ingress or debris are inspected.
- Test the low-pressure side. Supply pressure and volume are measured under the conditions specified for that engine, along with lift-pump power, ground, command, and controller operation.
- Test the high-pressure side. Rail pressure, pressure-control valves, volume-control valves, injector leakage or return, and pump output are evaluated against current Ford specifications.
- Inspect electrical circuits. Harnesses, connectors, fuses, grounds, and pump-cover or gasket circuits are checked before condemning a mechanical component.
- Apply the correct service procedure. The technician matches the findings to current Ford workshop information, technical service bulletins, and the vehicle identification number.
Ask for the measured values and the reason each replaced part was necessary. A professional diagnosis should distinguish a failed low-pressure pump, a high-pressure pump problem, a restriction, a control fault, and contamination.

A Ford technical-service summary describes 2011–2016 6.7L vehicles as using a low-pressure fuel pump and filter assembly called the fuel conditioning module. Starting with 2017, the low-pressure pump is in the tank, and later vehicles use a fuel filter and water separator arrangement that differs from earlier designs. Because that summary is a secondary reproduction and layouts vary by application, verify the current Ford information for the VIN before ordering parts or approving work.

Never assume that a filter, pump module, wiring connector, or flushing procedure from one Super Duty generation fits another. Engine displacement, chassis configuration, emissions equipment, and production changes can alter the correct part and test sequence.

These are diagnostic branches, not interchangeable do-it-yourself options. The reviewed Ford material does not establish universal labor times, repair prices, or one repair choice for every truck.
Warranty, insurance, and documentation
Coverage depends on the vehicle, cause, jurisdiction, warranty terms, and insurance policy. Ford South Africa’s consumer guidance states that contamination-related repairs are not covered by its Express New Vehicle Warranty and notes that insurance may apply. That statement is specific to that market and should not be generalized to another country or policy.
Before authorizing work, request:
- The diagnostic findings and stored codes.
- Photos or descriptions of water, wrong fuel, DEF residue, or metal debris.
- The Ford procedure or bulletin being applied, including its model-year and engine applicability.
- A list of parts to be replaced, flushed, or inspected.
- Separate parts and labor warranty terms for the repair.
- Copies of fuel receipts, towing records, and the repair order if an insurance claim may be involved.
When a replacement fuel filter is appropriate
A vehicle-specific diesel filter or fuel-conditioning component can be appropriate for scheduled maintenance, a confirmed restriction, or a technician’s documented water-in-fuel finding. Verify model year, engine, cab or chassis configuration, and VIN/catalog fitment. A replacement filter is not a repair for a failed high-pressure pump, does not neutralize gasoline or DEF contamination, and is not a universal fit part.
