A W212 (2009–2016) is mechanically and electronically different from a W213 (introduced in 2016). On a W213 OM654 diesel, insist on a genuinely cold start and investigate any top-end rattle; on either generation, verify gearbox servicing, diesel-emissions repairs, suspension specification and recall completion before agreeing a price.

Start with the generation, body style and engine
“E 220d” is a badge, not one single specification. The W212 generation covers 2009–2016, while the W213 starts in 2016. On W212 cars, the saloon is coded W212, the estate S212 and the coupé C207. Early cars used related E 220 CDI, BlueTec or BlueEfficiency naming before the “d” suffix became standard. The W212 buying guides describe a 2.1-litre four-cylinder diesel in the common E 220 CDI/E 220d range. The later W213 E 220d uses the 2.0-litre, four-cylinder OM654 diesel, listed at about 194 hp in a 2026 UK guide.

Model-year and registration overlaps mean you should decode the VIN and read the build specification rather than relying on the registration year or badge. An estate can have different weight, wheel and suspension equipment from a saloon with the same engine name.

Is a used E 220d reliable?
There is no sufficiently documented, directly attributable reliability statistic that can rank every E 220d. The available buying guides describe the W212 as broadly above average for its class but expensive to repair, while the W213 is presented as a strong default choice only when maintenance and gearbox-fluid records are complete. Those are buying-guide judgements, not a controlled comparison or a guarantee.
Condition, time-ordered invoices and evidence that known faults were repaired are more useful than a low odometer reading. A high-mileage car with regular servicing, correct oil and documented emissions or suspension work can be a lower-risk purchase than a low-mileage car with gaps, short journeys and warning lights recently cleared. Treat the post-2020 W213 facelift as a preference expressed by one 2026 guide, and the W212 facelift as a value option suggested by another—not as universal reliability rules.
What to check when viewing an E 220d
1. Verify identity, history and recall status
- Record the VIN, registration, mileage and exact body style. Match every service entry and invoice to the car’s dates and odometer readings.
- Ask for the complete service history in chronological order, not just a stamped booklet. Look for the engine-oil specification, routine inspection work, and invoices for injectors, AdBlue/SCR, NOx sensors, suspension or electronic faults.
- Confirm gearbox-fluid servicing against the schedule for that exact transmission. The guides stress fluid history but do not establish one interval that applies to every E-Class variant.
- Run a VIN-specific recall check through the UK DVSA recall service and ask the seller or a Mercedes dealer for completion evidence. Campaigns differ by vehicle and can change over time.
- Compare the asking price with an independent inspection estimate. A seller who will not provide the VIN, allow a cold start or permit diagnostic inspection is offering too much uncertainty.
Historical W212 guides also recommend checking recall status and injector history. Do not transfer their old tax, servicing or repair prices to a 2026 purchase.
2. Insist on a cold start
Arrange to see the car before it has been warmed up. On a W213 OM654, listen from outside the car and with the bonnet open for a metallic top-end rattle during the first seconds after starting. A 2026 buyer’s guide associates that sound in some higher-mileage examples with reported camshaft or cam-follower wear; it is not a fault on every OM654. Ask for invoices showing the correct oil specification and any top-end investigation. A persistent rattle, an unexplained warm engine or no oil documentation should trigger a specialist inspection or a decision to walk away.
Look around the camshaft cover for oil seepage. Let the engine idle until stable, check the instrument cluster for stored or active warnings, and make sure the cooling fan, air conditioning and charging system behave normally.

3. Test the diesel emissions system
AdBlue/SCR and NOx faults can be expensive because the system includes a tank, heater, sensors and control electronics. Review diagnostic history and invoices for AdBlue tank or heater work, NOx sensors and injectors. Some early W213 diesels are reported with these problems, and a dashboard warning that has just been reset is not proof of a repair.
On a W212 diesel, concentrate on hesitation, judder or uneven acceleration and ask specifically for injector history. A smooth test drive does not rule out an intermittent fault, so a diagnostic scan and invoice review are worthwhile.

With the engine and transmission at operating temperature, select reverse and drive several times, then make gentle and firm accelerations. Engagement should be prompt and free of harsh clunks, flare or shudder. During the drive, use manual selection or the available modes to check that changes occur when commanded. The key evidence is not a claimed lifetime fill; it is a documented fluid service performed to the correct schedule for that car’s gearbox.
5. Establish whether it has steel suspension or AIRMATIC
Do not assume every E 220d has the same suspension. If AIRMATIC air suspension is fitted, park the car on level ground, photograph the ride height, leave it overnight and compare the heights the next day. A corner sitting low indicates a possible air leak or strut problem. Listen for a compressor that runs for an unusually long time or repeatedly cycles, and request invoices for struts, compressor, valve-block or leak repairs.
Large wheels can make the ride firmer and increase tyre-replacement cost. Inspect the tyre shoulders, tread depth and matching brands, then price replacement tyres in the fitted size before treating a cheap car as a bargain.

6. Exercise every electronic system
Allow enough time to operate the infotainment system (COMAND or MBUX, depending on generation), central display, navigation if fitted, reversing camera, parking sensors, radar and driver-assistance functions, climate controls, electric seats, windows, mirrors, lights and keyless or battery-dependent features. Confirm that menus load, cameras are clear and no warning remains after a restart. A diagnostic scan should be used to investigate stored faults rather than simply clearing them.

7. Inspect wheels, body and underbody
Look for corrosion, buckling and kerb damage on alloy wheels; a W212 guide specifically flags wheel condition. Check panel gaps, paint variation, moisture in lamps, windscreen chips, door and boot seals, exhaust components and signs of underbody impact. Confirm that the spare-wheel or tyre-repair equipment, locking-wheel-nut key and charging or auxiliary cables supplied with the car are present.
8. Arrange an independent inspection
An independent Mercedes specialist can combine a road test, lift inspection and diagnostic scan. This is especially important where AIRMATIC, diesel-emissions warnings, an unexplained cold-start noise or incomplete records are involved. Agree in writing what the inspection covers and obtain a repair estimate for every finding before negotiating.
Common W213 E 220d problems
OM654 top-end wear
Reported camshaft and cam-follower wear is most relevant to higher-mileage examples showing a cold-start top-end rattle. It is not a universal OM654 defect. Correct oil, regular servicing and a quiet cold start reduce uncertainty; they do not replace a mechanical diagnosis if noise is present. One 2026 UK cost guide gives about £1,500 or more as an indicative example for an OM654 top-end repair, not a fixed quotation.
AdBlue, NOx sensors and injectors
Review the car’s diagnostic printouts and invoices for SCR/AdBlue tank or heater faults, NOx sensors and injector replacement. Warning messages can lead to starting restrictions or limp-home operation if ignored. Verify that a repair was completed and the fault codes stayed cleared after a road test.

Electronics and driver aids
W213 cars can have extensive cameras, radar, displays and driver-assistance hardware. Test each feature and inspect the windscreen and bumpers for repairs that could affect sensors. A fault-free screen at the viewing is not enough if the history shows repeated warning codes.
Common W212 E 220d problems
Injector hesitation or judder
Historical W212 buying guidance identifies diesel injector hesitation and judder as a weak spot. Reproduce the symptom with gentle acceleration from low revs and under load, then request injector test results or replacement invoices. A specialist should distinguish injector trouble from transmission, engine-mount or emissions faults.
Age-related wheels and equipment
Inspect alloy wheels for corrosion and cracks, particularly on older cars. Check the full electrical load, including climate control, displays, parking sensors and cameras where fitted. Older navigation or media units may function differently from newer W213 systems, but any warning, dead pixel or intermittent control should be priced before purchase.
Recalls and deferred maintenance
Because the earliest W212 cars are now well over a decade old, missing services, long periods off the road and previous owners’ deferred repairs matter more than trim level. Match mileage to MOT and service dates, and verify recall completion by VIN.

A separate guide quotes AIRMATIC estimates of $1,500–$3,000 per corner and more than $5,000 for a complete system, but those are US-dollar figures in a different market and should not be converted into a UK quote. Their practical lesson is that suspension specification and maintenance records can materially change ownership risk.
How to compare two E 220d cars
When two cars appear similar, compare them in this order:
- Generation and engine: establish W212 versus W213 and the exact diesel specification.
- Documentation: favour complete, time-ordered servicing, correct-oil evidence and gearbox-fluid records.
- Known repair history: compare injector, AdBlue/SCR, NOx, camshaft, suspension and electronic invoices.
- Cold-start condition: reject unexplained top-end noise, oil leaks or warning messages until a specialist has diagnosed them.
- Suspension: price AIRMATIC risk separately from a car on conventional springs.
- Equipment and tyres: confirm that cameras and driver aids work, and account for wheel size, ride comfort and tyre cost.
- Use case: choose saloon or estate according to load space and consider whether your annual driving pattern suits a modern diesel.
- Total deal: deduct the independent inspection’s immediate repair estimate from the apparent price advantage.
A newer W213 offers newer equipment and systems; a W212 facelift can provide value. The evidence does not support declaring one generation categorically reliable or unreliable, so let condition and records decide.
Questions to ask the seller
- Can I see the VIN and the complete service file before arranging a deposit?
- When was the gearbox fluid last changed, and which schedule was followed?
- Has the car had AdBlue tank, heater, NOx-sensor or injector work?
- Does it have AIRMATIC, and are there invoices for struts, leaks or the compressor?
- Can I start it from cold without the engine being warmed first?
- Are there any current or recently cleared warning lights?
- Which recalls apply to this VIN, and can you show completion evidence?
- Will you allow an independent Mercedes specialist to perform a diagnostic and lift inspection?
Bottom line
Buy the documented car, not the badge. Decode the generation and engine, witness a cold start, verify oil and gearbox servicing, investigate AdBlue/NOx and injector history, test every electronic aid, identify the suspension type and run a VIN recall check. A post-2020 W213 may suit a buyer seeking newer equipment, while a well-kept W212 facelift can make financial sense; neither choice removes the need for a vehicle-specific inspection and current repair quotes.
