Hulu account data showed continuous video streaming through 9:59 p.m. on the night an Uber test SUV fatally struck a pedestrian in Tempe, Arizona, at about 9:58 p.m. The NTSB’s formal finding was not that Hulu alone caused the crash: it said the vehicle operator failed to monitor the road and automated driving system because she was visually distracted by her personal phone. The investigation also identified failures in Uber’s safety practices and weaknesses in oversight of automated-vehicle testing.
What the Hulu record shows
The NTSB docket’s human-performance report says Hulu account data showed continuous video streaming from 9:16 p.m. through 9:59 p.m. on March 18, 2018. That interval overlaps the crash, which occurred at approximately 9:58 p.m. The report also notes that Uber prohibited phone use while operating a self-driving vehicle. NTSB investigation and docket
The account record establishes that streaming was occurring during the relevant period; it does not establish that viewing a particular show, by itself, caused the crash. The NTSB’s probable-cause wording focuses on visual distraction from the operator’s personal phone and her failure to monitor the driving environment and automated driving system (ADS).
How the crash happened
At 9:58 p.m. on March 18, 2018, a modified 2017 Volvo XC90 operated by Uber Advanced Technologies Group (ATG) struck a pedestrian crossing northbound Mill Avenue in Tempe. The vehicle was on a public-road test route under control of Uber’s developmental ADS, with a human operator in the driver’s seat. The pedestrian died; the operator was not injured. The system detected the pedestrian but did not respond in time
The ADS first detected the pedestrian 5.6 seconds before impact and tracked her, but it did not accurately classify her as a pedestrian or predict her path. The ADS recorded the SUV traveling at 45 mph as it approached the collision site. Its design precluded emergency braking for collision mitigation and relied on the operator to intervene. By the time the ADS determined a collision was imminent, the situation was beyond the system’s braking-response specifications.
The operator looked toward the road about one second before impact and began steering left just before the collision. The NTSB’s account therefore describes a breakdown involving both human monitoring and the system’s detection, prediction, and response—not a vehicle that never detected the pedestrian.
What the NTSB said caused the crash
The NTSB identified the operator’s failure to monitor the driving environment and ADS because of visual distraction as the probable cause. It also named several contributing factors, which the Board treated as part of the crash’s explanation rather than as competing alternatives to the probable cause:

- Uber ATG’s safety practices: inadequate safety risk assessment procedures, ineffective oversight of vehicle operators, and inadequate countermeasures for automation complacency, linked by the NTSB to an inadequate safety culture.
- The ADS design: the system did not accurately classify the pedestrian or predict her path, and its design did not provide emergency braking for collision mitigation.
- The pedestrian’s crossing: the NTSB identified her impaired crossing outside a crosswalk as a contributing factor.
- Government oversight: the Board identified insufficient Arizona oversight of automated-vehicle testing as a contributing factor.
The NTSB’s final report described the operator’s prolonged visual distraction as a typical effect of automation complacency. Why the change from two operators mattered
About five months before the crash, Uber ATG had changed its operation from two vehicle operators to one. The second operator had provided a layer of safety redundancy; removing that person left the sole operator with more duties, including interacting with the human-machine interface. That context helps explain why the NTSB examined operator workload and company oversight, not only phone use.
When testing restarted in December 2018, ATG returned to two operators, expanded operator training, and added an inward-facing camera system. The system could alert the operator and supervisor if the driver-seat operator looked away from the road for several seconds. What changed in the safety recommendations
The NTSB recommended safety self-assessments and evaluation for companies testing automated driving systems on public roads, state applications and expert review addressing safety risk management, and completion of Uber ATG’s safety management system. In its automation summary updated December 4, 2024, the Board listed the recommendation concerning Uber ATG’s safety management system as closed on April 26, 2024.
The same NTSB summary cautions that safety self-assessments are voluntary and are not evaluated by the National Highway Traffic Safety Administration (NHTSA), and says federal safety risk-management requirements for public-road automated-vehicle testing are absent. The status describes the oversight landscape reported by the NTSB as of December 4, 2024; it should not be read as a general accident-rate finding or as a measure of the safety of automated vehicles overall.
