How to position each part of the belt
Lap belt: low, snug and below the belly
Place the lap portion across your hips and pelvic bone, below your abdomen. It should lie snugly against your body rather than crossing the belly or riding up over it. Buckle the belt and pull out looseness so the lap section stays in that low position. ACOG’s instruction is to buckle it “below your belly” so it fits snugly across the hips and pelvic bone; NHTSA likewise directs pregnant occupants to wear it low across the hips and below the belly.
Check the route after you buckle: the belt should cross the bony hip area, not the soft area over the abdomen. A lap belt placed over or on top of the belly is not the recommended fit, even if that position initially feels easier. If the belt is loose, remove the slack and check that it remains low. It should not be allowed to ride up while you are seated.

Shoulder belt: across the chest and collarbone
Route the shoulder portion diagonally across the middle of your chest, between your breasts, and over the middle of your collarbone. It should not press against your neck. Keep it over your shoulder and chest: do not tuck it under an arm or move it behind your back. Those alternate routes are not correct substitutes when the belt feels awkward.
Look at both the starting point at your shoulder and the diagonal line across your chest. The belt should be on the middle of the collarbone, not at the edge of the shoulder, and should remain away from the neck. Do not change the route to relieve contact or make the belt feel more convenient; instead, adjust your seated position and check the fit again.
Check the fit after buckling
- Fasten the belt and confirm the lap portion crosses the hips below the belly.
- Check that the shoulder portion crosses the chest and collarbone, away from the neck.
- Pull out slack and check again that the lap portion has not shifted upward.
- Confirm that the shoulder belt remains over the shoulder and chest, rather than under an arm or behind your back.
- Before moving, settle into the position in which you will travel and make a final check that the belt still sits correctly.
The belt must stay in these positions while you are seated, not just look correctly placed at the moment you buckle it. If it shifts, adjust it before the vehicle moves. Recheck it whenever you change your seating position. Correct placement is a combination: the lap section stays low and snug, the shoulder section follows the chest and collarbone, and the belt has no slack. A good fit does not mean wearing only one section of the belt.
Adjust the seat and steering wheel as a driver
Use an upright, comfortable seating position. Move the seat back as far as is practical while still allowing you to reach and operate the steering wheel and pedals comfortably. Avoid unnecessary recline, and do not let your belly touch the steering wheel. ACOG recommends at least 10 inches between the steering wheel and your breastbone. NHTSA’s general air-bag guidance also recommends a minimum 10-inch distance between the air-bag cover and breastbone; maintain that distance only while retaining proper position and control of the vehicle. See NHTSA’s air-bag guidance.
Set the seat and steering wheel together rather than sacrificing comfortable control to create more distance. The 10-inch measure is from the breastbone, not the belly. If your vehicle’s adjustment range does not let you sit comfortably and operate the controls while keeping the wheel away from your body, consult the vehicle owner’s manual or manufacturer for vehicle-specific options. Do not solve the problem by reclining substantially or changing the belt’s route.
A practical sequence is to sit upright first, move the seat back while checking that you can still operate the pedals and steering wheel comfortably, and then recheck the belt across your hips and chest. The distance guidance is not a reason to sit so far away that you cannot control the vehicle. Likewise, comfortable control is not a reason to let the belly touch the wheel or to recline unnecessarily. If you cannot reconcile the fit and control in your vehicle’s adjustment range, ask the manufacturer or consult the manual rather than improvising a different belt route.
Wear the belt with the air bags on
Do not turn off the air bags because you are pregnant. NHTSA explains that air bags are designed to work with seat belts, not replace them. The belt remains necessary, and the air bags are part of the vehicle’s restraint system. Keep the belt positioned correctly and follow the vehicle’s instructions for seat and steering-wheel adjustment.

This means the two measures go together: buckle the lap-and-shoulder belt correctly and leave the air bags on. An air bag is not a substitute for buckling up, and the presence of an air bag is not a reason to put the belt under an arm, behind the back, or over the belly. NHTSA’s air-bag distance recommendation also depends on keeping a position that permits proper operation of the vehicle.
Passenger seat: use the same belt fit
Whether you are driving or riding as a passenger, use the lap-and-shoulder belt and keep its two sections in the positions described above. If you are in the front passenger seat, adjust the seat to a comfortable upright position and move it back as far as the vehicle permits while preserving a proper belt fit. Follow the vehicle manual for any seat-specific instructions. Pregnancy does not make the shoulder belt optional, nor is it a reason to route the lap belt over the abdomen.
Do not change the belt route just because you are not driving. The lap portion still belongs low across the hips and pelvic bone, and the shoulder portion still belongs across the chest and middle of the collarbone. Make sure the belt is snug and that you have not placed anything between yourself and the belt. For vehicle-specific seat adjustments, use the instructions for that vehicle.
If the belt feels uncomfortable or does not fit properly
First adjust your seating position and recheck the belt placement. A vehicle manual excerpt hosted in NHTSA’s document repository advises adjusting the seating position rather than wearing the belt incorrectly and warns against placing anything between the occupant and the belt. That excerpt is from a Tesla Model S manual, so treat it as vehicle-specific corroboration, not a substitute for the manual for your own car. Read the manual excerpt.

Then adjust the seat while maintaining comfortable control if you are driving. Do not respond to discomfort by routing the shoulder belt under your arm or behind your back, or by moving the lap belt onto the belly. Those changes depart from the ACOG and NHTSA guidance.
If you cannot get the standard belt to fit correctly after adjusting the seat, consult your vehicle’s owner manual and contact the vehicle manufacturer or dealer for guidance specific to that vehicle. NHTSA’s general seat-belt page discusses contacting a manufacturer about fit issues, but that generic advice is not an endorsement of pregnancy belt-positioning accessories. Do not assume a positioner, extender or other aftermarket device will improve pregnancy fit.
Why an accessory is not the default fix
ACOG and NHTSA explain how to position the installed belt; they do not say a separate device is necessary to hold it in place. In a historical interpretation concerning the described aftermarket “Belly Safe” device, NHTSA said it was unaware of a need for the device when the belt is adjusted as recommended and raised concerns about slack, crash forces and submarining. That interpretation is about the device and circumstances it addressed; it is not a current test or blanket certification of every product. Read NHTSA’s interpretation.

The distinction matters: adjusting the existing belt as directed is different from adding a device that changes how the belt sits or is secured. NHTSA’s interpretation does not establish that every product has been tested or that every vehicle will fit every occupant in the same way. It does, however, mean the cited agencies do not establish a pregnancy belt positioner as a required part of correct belt use. If you have a fit problem, start with the vehicle manual and manufacturer rather than treating an accessory as an automatic solution.

NHTSA’s general belt guidance also discusses contacting a dealer about adjusters or contacting a manufacturer about extenders for fit problems. That general fit information should not be read as approval of pregnancy-positioning devices. No accessory is specified as necessary in the ACOG and NHTSA pregnancy instructions. The vehicle manufacturer is the appropriate place to ask about a particular vehicle’s belt fit and any vehicle-specific options.
After a crash, get medical attention immediately
Seek medical attention immediately after any crash, even if you feel uninjured or have no obvious injuries. Both ACOG and NHTSA advise prompt medical attention after a collision during pregnancy. Do not use the absence of pain or visible injury as a reason to wait. The advice applies even if the belt appeared to stay in place and even if the collision seemed minor to you.
Do not try to decide on your own that an evaluation is unnecessary because you feel well. ACOG and NHTSA’s guidance is to obtain medical attention after a crash during pregnancy. Keep this response separate from the routine fit check: seat and belt adjustments help you travel correctly, but they do not replace medical attention after a collision.
Why correct belt use matters—and what the available numbers do not show
ACOG says a pregnant person and fetus are much more likely to survive a crash when the pregnant person is buckled. NHTSA calls buckling up through all stages of pregnancy the single most effective action for protecting the pregnant person and unborn child in a crash. The official guidance does not provide a pregnancy-specific numeric estimate of crash risk or fetal outcomes, so general crash statistics should not be mistaken for pregnancy-specific measurements.
For context only, NHTSA reports that 22,713 passenger-vehicle occupants were killed in 2024 and that 48% of those killed with known seat-belt-use status were unrestrained. Those are general U.S. passenger-vehicle figures, not pregnancy-specific findings. The same NHTSA page cites an estimate of 14,955 lives saved by seat belts in 2017; that is an older, year-specific estimate, not a current-year count. NHTSA’s seat-belt page provides the agency’s figures and guidance.
These figures provide context for seat-belt use in passenger vehicles generally. They do not measure the risk to a pregnant occupant or fetus, and they cannot be used to calculate an individual pregnancy’s outcome after a crash. Keep the central guidance practical: buckle up every trip, position the belt correctly, and seek medical attention after a collision.

- Lap belt is low and snug across the hips and pelvic bone, below the belly.
- Shoulder belt crosses the chest and middle of the collarbone, away from the neck.
- Shoulder belt is over the shoulder—not under an arm or behind the back—and slack has been removed.
- Driver is upright, can comfortably operate the controls, and has moved back as far as practical without losing control.
- Air bags remain on and are used together with the seat belt.
- If riding as a passenger, the same lap-and-shoulder placement is in use and the seat is adjusted according to the vehicle’s instructions.
What to do if you cannot get the fit right
Use this order rather than changing the belt route: settle into an upright position, buckle the standard belt, place the lap section low across the hips below the belly, route the shoulder section across the chest and collarbone, and remove slack. If driving, adjust the seat and steering wheel so you can operate the controls comfortably while keeping as much distance from the wheel as practical. Then check the belt placement again.
If those adjustments do not give you a proper fit, consult the owner’s manual or contact the vehicle manufacturer or dealer. Vehicle adjustment ranges and instructions are specific to the vehicle, so advice for one model does not automatically apply to another. Avoid placing anything between you and the belt, and do not assume an aftermarket product is an approved or necessary solution. If a collision happens, seek medical attention immediately regardless of whether you noticed an injury.

Should I turn off the air bag while pregnant?
No. Keep the air bags on and wear the seat belt. NHTSA says air bags are designed to work with seat belts, not replace them.
Do I need a pregnancy seat-belt positioner?
ACOG and NHTSA do not identify a separate positioner as necessary when the installed belt is adjusted correctly. NHTSA’s historical interpretation concerning the Belly Safe device raised concerns about slack, crash forces and submarining. For fit problems, consult the vehicle manual or manufacturer rather than assuming an aftermarket device is appropriate.
What should I do if the belt does not fit comfortably?
Adjust your seating position and recheck the belt rather than wearing it incorrectly or placing anything between you and the belt. If the standard belt still does not fit properly, consult your vehicle’s owner manual or contact the manufacturer or dealer for vehicle-specific advice.

What should I do after a crash if I feel fine?
Seek medical attention immediately, even if you feel uninjured or have no visible injuries. ACOG and NHTSA advise prompt medical attention after a collision during pregnancy.
Does NHTSA’s crash data show pregnancy-specific risks?
No. NHTSA’s figures of 22,713 passenger-vehicle occupant deaths in 2024 and 48% unrestrained among those with known belt-use status are general vehicle-occupant statistics. They do not quantify pregnancy-specific or fetal outcomes.
The Bottom Line
For every trip, wear the lap belt low and snug across the hips below the belly, with the shoulder belt across the chest and collarbone. Stay upright, adjust the seat for comfortable control, keep air bags on, and seek medical attention immediately after any crash. If the standard belt does not fit correctly, ask the vehicle manufacturer or consult the owner’s manual rather than assuming an aftermarket positioner is needed.
