Ford’s BlueCruise driver-assistance system is under investigation by the National Highway Traffic Safety Administration (NHTSA) following two 2024 fatal crashes that killed three people. NHTSA upgraded its review to an Engineering Analysis in January 2025; separately, the National Transportation Safety Board (NTSB) identified driver overreliance in both crashes and cited different contributing factors in each. Neither the NHTSA investigation nor the NTSB findings amount to a NHTSA defect determination or recall.
Why the headline says two crashes but three people died
The scrutiny centers on two crashes involving 2022 Ford Mustang Mach-E vehicles operating with BlueCruise: one in San Antonio, Texas, and one in Philadelphia, Pennsylvania. One person died in San Antonio and two in Philadelphia, for three deaths in total. NHTSA’s June 2025 information request described two fatal collisions among four frontal collisions then known to the agency; that was its count at that time, not a claim that no other incidents existed.
What happened in the two crashes

According to the NTSB’s March 2026 release, neither driver nor system applied braking or steering in the moments before impact in either crash. The board said driver overreliance contributed to both. These are NTSB findings, not NHTSA conclusions.
What the federal investigation means
- April 2024 — Preliminary Evaluation: NHTSA opened PE24012 after notice of two fatal collisions involving BlueCruise-equipped Mustang Mach-Es. Its opening summary said BlueCruise was engaged immediately before both crashes and described it as partial driving automation requiring active driver supervision. NHTSA investigation materials.
- January 17, 2025 — Engineering Analysis: NHTSA upgraded the review to EA25001 to investigate system limitations further and assess drivers’ ability to respond to scenarios beyond those limits. An Engineering Analysis is an investigative stage, not a recall or final finding that a defect exists.
- June 18, 2025 — Information request: NHTSA asked Ford for information. The letter said the agency was then aware of four frontal collisions, including two fatal ones, and set out a broad group of subject and peer vehicles for information gathering.
- March 31, 2026 — NTSB conclusions and recommendations: At a public board meeting, the NTSB announced its findings and called for stronger performance requirements and oversight for partial automation, crash-data recording and notification requirements, improved driver monitoring, and BlueCruise changes addressing driver attention and excessive speed.
The agencies have different roles: NHTSA is investigating whether a vehicle or system may present a safety-related defect; the NTSB investigated the two crashes and issued probable-cause findings and recommendations. The cited NHTSA materials describe further investigation, not a completed or closed case. The NTSB’s March release said a final report would follow in several weeks; the release alone does not establish whether that report had been published by October 8, 2026.

Does BlueCruise stop for stationary cars?
The San Antonio crash shows that a driver should not assume BlueCruise will detect and stop for a stationary vehicle: NTSB found that the system, including automatic emergency braking, did not detect and respond to the Honda in that crash. That finding does not establish that the system will always fail to respond, or how it performs in every situation. Ford’s owner manual states that BlueCruise “is not a crash avoidance system.”

No. NHTSA describes BlueCruise as partial driving automation, and says the driver is expected to actively supervise its performance. Ford’s manual describes hands-off steering only in certain situations on compatible roads, called blue zones, when adaptive cruise control is in use. Hands-off steering does not mean the vehicle can be left to drive itself.

Ford’s U.S. owner-manual page, marked copyright 2026, lists limitations involving lane position and markings, merging or splitting lanes, curves, construction areas, aftermarket steering modifications, spare-tire use, driver-camera visibility, weather, direct sunlight, and potentially inaccurate or outdated navigation maps. It also warns that steering assistance is limited and may not be sufficient for every situation. Owners should consult the manual for their exact vehicle year and version rather than assume every listed detail applies identically to every Mach-E.

What drivers should take from the findings
- Keep supervising the road and be ready to steer or brake; BlueCruise does not replace an attentive driver.
- Do not rely on driver assistance to overcome impairment or distraction. The NTSB identified those factors in its Philadelphia findings.
- Pay particular attention to work zones, stationary traffic, lane changes, curves, and other conditions Ford identifies as limitations.
- Do not treat a system’s hands-free operation as evidence that it can prevent every collision.
