Turbocharger and sensor reports are concentrated in early trucks, Ford’s documented driveability bulletin covers only a defined 2011–2014 build group, and a reported CP4.2 high-pressure-pump failure can contaminate an entire fuel system. A symptom, warning message, or diagnostic trouble code is a starting clue—not proof that a particular part has failed.
Before choosing a repair, record the truck’s model year, build date, mileage, stored codes, exact dash message, maintenance history, fuel quality, and modifications. Then follow a year-specific diagnostic procedure. The guide below separates reported generation-specific concerns from Ford’s owner-maintenance requirements and documented service information.

- Model year and build date: Early 2011–2012 turbo reports, first-generation emissions sensors, and Ford’s 2011–2014 bulletin do not automatically apply to later trucks.
- Exact symptoms: Note whether the engine cranks, starts, stalls, surges, shudders, knocks, overheats, or enters a speed-limited or idle-only mode.
- Messages and codes: Photograph the warning message and save all powertrain, fuel, emissions, and network codes before clearing them.
- Service history: Verify both fuel filters, water-separator draining, oil changes, coolant maintenance, DEF service, and prior emissions repairs.
- Operating conditions: Record towing load, ambient temperature, regeneration status, fuel source, and whether the problem began after maintenance or a modification.
- Vehicle identification: Check the VIN for open recalls and superseding service information. A bulletin’s existence does not prove that a particular truck qualifies.
There is no published failure-rate percentage in the cited material. Descriptions such as “reported weak point” or “common problem” should not be converted into a prediction that every truck will fail.

High-pressure fuel pump (CP4.2) and fuel-system contamination
MotorTrend identifies the Bosch CP4.2 pump as a reported weak point on 6.7L engines before 2020. If a pump breaks down internally, metallic or other debris can move through the high-pressure circuit and damage injectors, rails, lines, and related components. That makes a confirmed pump failure a potentially much larger repair than a pump-only replacement. The source does not provide a failure rate, a universal symptom list, or evidence that an additive or aftermarket kit prevents failure.
Symptoms that warrant fuel-system testing
Hard starting, a no-start, rough running, loss of power, stalling, unusual pump noise, or a warning lamp can occur with many faults, including low fuel pressure, contaminated fuel, electrical problems, injector faults, or control-system issues. None proves CP4.2 failure. A technician should test supply and high-pressure operation using the procedure and specifications for that model year, inspect fuel samples and filters for contamination, and scan for stored and pending codes. Avoid repeated cranking while the cause is unknown.
What to do when contamination is suspected
- Stop operating the truck if it is losing power, making abnormal high-pressure-pump noise, or showing evidence of metal in the fuel system.
- Do not authorize a pump-only repair until the shop explains how it will inspect and decontaminate the rest of the system.
- Have the fuel filters, drained fuel, pump, rails, lines, injectors, and tank evaluated according to Ford’s service procedure.
- Ask for photographs or test results that distinguish metallic wear debris from ordinary dirt or water contamination.
- After repair, confirm that the shop completed required priming, air removal, software checks, and post-repair pressure verification.
Do not continue extended cranking after the truck runs out of fuel. Ford warns that prolonged cranking in that situation can damage the high-pressure pump.
Fuel filters and the water separator
The diesel fuel system uses two filters: a frame-mounted primary fuel-and-water separator and an engine-mounted secondary filter. Ford directs owners to replace both at the specified maintenance interval and to drain the separator regularly or whenever the vehicle indicates it. The exact interval varies by edition, so use the owner manual for the truck’s year and VIN rather than a generic schedule.
Ford’s owner guidance states: “Proper fuel filter maintenance and prompt water draining when the water in fuel light illuminates is essential to prevent injection equipment damage.” That sentence comes from Ford’s edition-specific 6.7L diesel owner-manual guidance, not from an independent failure study.
Water-in-fuel warning and reduced power
When the water-in-fuel lamp or message appears, safely stop and drain the separator as the manual directs. If the warning remains, returns quickly, or is accompanied by reduced power, have the fuel sampled and the system diagnosed. Ford warns that ignoring the warning can lead to reduced-power mode and injection-equipment damage. Do not assume that changing one filter clears the underlying problem; both filters and the separator need attention.

Filter-service mistakes
- Installing only the engine-mounted filter while leaving the frame-mounted separator overdue.
- Failing to drain the separator after contaminated fuel or a warning.
- Allowing dirt into an open housing or using an incorrect filter design.
- Cranking repeatedly instead of following the manual’s priming procedure.
- Clearing a warning without finding out why water or contamination entered the system.
Early turbocharger and coolant leaks
MotorTrend reports ceramic turbo ball-bearing concerns on 2011–2012 trucks and mentions early radiator and turbo coolant-inlet leaks. These are reported early-version issues, not a blanket defect covering every Scorpion engine. A turbo noise, low-boost complaint, smoke event, or coolant loss requires inspection of the turbocharger, charge-air plumbing, lubrication, and cooling circuits.
Check the coolant level only with the engine cold and use the specified coolant and additive requirements for that year. Ford warns that insufficient coolant can cause severe engine damage. A falling level, dried residue, visible leak, sweet exhaust odor, or repeated need to top up should be investigated before towing or heavy use. Do not substitute stop-leak products for diagnosis.
EGR soot buildup, P0401, and flow diagnosis
MotorTrend reports severe soot buildup in some EGR coolers and associates diagnostic trouble code P0401 with insufficient EGR flow. P0401 means the control system detected less EGR flow than expected; it does not prove that the cooler itself must be replaced.
Ford’s 2023 6.7L Power Stroke service-training material explains that exhaust can travel through the EGR cooler or through a bypass path, depending on operating conditions. A correct diagnosis therefore considers the complete EGR and air-management system: valves, actuators, passages, cooler, bypass path, sensors, wiring, intake restrictions, and software strategy.
A sensible P0401 diagnostic sequence
- Record all codes and freeze-frame information, including engine temperature, load, and commanded EGR state.
- Inspect connectors, harnesses, vacuum or actuator controls where fitted, intake plumbing, and obvious soot or exhaust leaks.
- Verify that the engine reaches the expected operating temperature and that related sensors report plausible values.
- Use the year-specific test to compare commanded and measured EGR flow through both cooler and bypass conditions.
- Inspect for restricted passages or a sticking valve only after electrical and sensor checks support that direction.
- Clear codes only after the repair, then complete the required drive cycle and confirm that the fault does not return.
Replacing an EGR cooler solely because P0401 is stored can waste money and leave a wiring, valve, sensor, or intake restriction unresolved.

NOx sensors and reduced-power complaints on early trucks
MotorTrend reports NOx-sensor failures through 2013 and says a failure may cause limp mode. Treat that as a generation-specific report, not a universal explanation for reduced power. A sensor code can result from the sensor element, wiring, connector, exhaust leak, calibration, or another emissions-system fault.

Capture the exact code and message, inspect the harness near heat and moving components, and follow the diagnostic test for that VIN. If the truck enters a speed-limited mode, avoid heavy towing until the cause is confirmed. Replacing a sensor without verifying the circuit and related emissions conditions can produce another warning and an unnecessary parts bill.
Ford TSB 15-0021: a narrow 2011–2014 application
Ford TSB 15-0021, published February 23, 2015, applies to some 2011–2014 F-Super Duty vehicles equipped with the 6.7L diesel and built on or before November 6, 2013. The bulletin itself warns that it may not apply to a particular vehicle. Confirm the build date and VIN, look for open recalls, and check whether Ford has issued superseding information.
For qualifying vehicles, the bulletin lists combinations of the following conditions:
- Malfunction-indicator-lamp illumination with specified diagnostic trouble codes.
- Intermittent harsh shifts, engine shudder, surge, or hesitation in a stated engine-speed range.
- Roughness or misfire during diesel-particulate-filter regeneration.
- DEF warnings that do not reset or delayed DEF-level updates.
- An abrupt shutdown feel.
- High-pressure-pump knocking noise at idle.
The repair path is the applicable Ford service procedure, not a generic parts replacement. The bulletin is intended for trained professional technicians and is not a do-it-yourself instruction. An independent diesel shop can still use the information, but it should verify the bulletin’s applicability and current Ford guidance before proceeding.
Reduced power: separate the warning from the cause
“Reduced power” describes a protective strategy, not one failed component. The trigger may be water in fuel, a fuel-pressure fault, an emissions fault, overheating, low coolant, a DEF condition, sensor disagreement, or another engine-protection event.
Immediate triage
- Read the exact message and note whether the truck limits speed, permits only idle, or simply changes throttle response.
- Check engine oil, coolant, fuel level, and DEF condition using the manual’s safety instructions. Do not open a hot cooling system.
- Scan for all codes, including pending and manufacturer-specific codes; save them before disconnecting the battery.
- Look for water-in-fuel indication, active regeneration, overheating signs, fluid leaks, unusual smoke, or abnormal pump noise.
- If the warning persists, the engine overheats, the oil-pressure warning appears, or the truck enters a severe limit, stop driving and arrange qualified diagnosis.
Ford says engine-protection systems can modify performance. Persistent warning lights or repeated reduced-power events should be checked by an authorized dealer or a qualified diesel technician.

Overheating and coolant-loss diagnosis
Ford’s troubleshooting list includes low or dirty coolant, blocked radiator or cooler fins, a malfunctioning fan drive, a sticking thermostat, heavy towing in hot weather, blocked airflow, a slipping belt, and a dirty air filter. This is a diagnostic list, not proof of one common engine defect.

- Verify coolant level and condition when cold; look for external leaks and dried deposits.
- Inspect radiator, charge-air cooler, and other cooling surfaces for mud, debris, bent fins, or airflow blockage.
- Check fan-drive operation, belt condition, and thermostat behavior using the year-specific service procedure.
- Consider towing weight, grade, ambient heat, and sustained regeneration as operating factors.
- Inspect the air filter and intake for restriction.
- Pressure-test the cooling system if the level falls or no external leak is visible.
Continuing to drive while the temperature warning is active can turn a repairable cooling fault into severe engine damage. Let the engine cool safely and follow the manual’s shutdown instructions.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.DEF warnings, speed limits, and idle-only mode
Ford says low or contaminated diesel exhaust fluid can lead to speed limitation or an idle-only mode. A DEF warning can also involve level sensing, quality detection, wiring, dosing, or an emissions-system fault. Read the exact message, verify the fluid is the correct product and not contaminated, and follow the vehicle-specific refill and restart procedure.
A delayed level update does not necessarily mean the tank is empty, while a full tank does not rule out a quality or dosing fault. If the warning does not clear after the manual’s prescribed action, obtain a scan and service diagnosis rather than repeatedly adding fluid or disconnecting the battery.
Maintenance that reduces avoidable risk
Use the maintenance schedule for the truck’s exact year and VIN. Core practices from Ford’s owner guidance are:
- Replace both diesel fuel filters at the specified interval.
- Drain the frame-mounted water separator regularly and whenever indicated.
- Do not continue extended cranking after running out of fuel.
- Change engine oil at the specified interval and use the required specification.
- Maintain coolant level and additive condition; do not extend service or mix products casually.
- Inspect cooling surfaces, belts, air filtration, and fan operation when towing or operating in dust and heat.
- Monitor DEF level and condition and respond to warnings before a speed limit or idle-only strategy begins.
- Keep emissions and regeneration faults from becoming chronic; repeated interrupted regeneration warrants diagnosis.
Ford warns that extending oil changes can affect performance, fuel economy, and engine life. It also warns that insufficient coolant or incorrect coolant additives can cause severe damage.

How to evaluate a used 6.7L Power Stroke
Do not rank model years by an unsupported reliability percentage. Compare the truck using evidence you can verify:
| Inspection area | Questions to ask | Evidence to request |
|---|---|---|
| Identity | What are the model year, build date, mileage, and VIN? | Door-label information, VIN report, and recall check. |
| Fuel system | When were both filters changed? Has water or metal been found? | Maintenance invoices, filter inspection, fuel-sample results if contamination is suspected. |
| Cooling | Has coolant been lost, overheated, or topped up repeatedly? | Cold-pressure test, leak inspection, service records, and evidence of correct coolant. |
| Emissions | Are there active or pending codes? How does regeneration behave? | Scan report, regeneration history where available, DEF records, and completed campaigns. |
| Driveability | Any shudder, surge, harsh shift, hesitation, abrupt shutdown feel, or idle pump knock? | Road test with scan data and technician assessment. |
| Modifications | Has the calibration, exhaust, intake, or emissions hardware been altered? | Parts list and documentation; return-to-stock work may be required for diagnosis. |
A cold start, hot restart, loaded road test, and scan before purchase reveal more than a brief idle demonstration. On an early truck, compare the build date with TSB 15-0021 rather than assuming the bulletin applies.
Repair decisions and common diagnostic mistakes
Replacing a part because a code names a system
P0401 identifies insufficient EGR flow, not automatically a failed cooler. A NOx code does not automatically condemn the sensor. A reduced-power message does not identify the pump, turbo, or DEF tank. Require test results that connect the code to the failed component.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsAssuming a reported issue affects every year
Early turbo and NOx reports, the pre-2020 CP4.2 discussion, and the narrowly bounded Ford bulletin have different scopes. Use the model-year table and build-date check before applying any claim.
Ignoring maintenance evidence
Overdue filters, an undrained separator, low coolant, extended oil intervals, or contaminated DEF can create expensive secondary damage. A maintenance record is diagnostic evidence, not paperwork alone.
Continuing to drive through protective warnings
Water-in-fuel, overheating, low coolant, persistent reduced power, and DEF limit messages should be addressed promptly. Continuing to operate under those conditions can increase damage or leave the truck stranded in a more restrictive mode.
Bottom-line diagnostic approach
The most defensible way to troubleshoot a 6.7 Power Stroke Scorpion is to identify the exact year and build date, preserve the codes and messages, verify maintenance and fluid condition, and test the complete affected system before replacing parts. Early trucks deserve targeted checks for the reported turbo, coolant, NOx, and Ford-bulletin conditions; pre-2020 trucks warrant informed awareness of CP4.2 consequences without assuming failure; and every year benefits from correct dual-filter service, water-separator draining, oil and coolant maintenance, and prompt response to fuel, temperature, emissions, and DEF warnings.
