Which engines appear in the ranking?
Gadget Review published “20 Truck Engines Ranked From Worst to Best” on October 7, 2025. The positions and engine-specific comments below reflect what that article identifies; they are not independently established failure rates or service-life findings.

How much should you trust the order?
Gadget Review says it considered power, reliability, maintenance requirements, and real-world performance across commercial, off-road, and heavy-duty applications. It does not publish a scoring method, comparable failure-rate data, or a way to normalize engines across those different classes and uses. The placements therefore represent that publisher’s judgment, not a statistically supported finding that one engine is more reliable than another.

The article’s comments about the DT466E and 6.4L Power Stroke are reported criticisms, not quantified comparisons backed by repair records in the accessible text. Likewise, the C12 mileage statement is an unsourced claim from the article—not a verified service-life benchmark. The ranking provides no named expert quotation or reliability statistic that would establish otherwise.
Why truck class, payload, and duty cycle matter
A ranking that puts engines from different kinds of trucks on one scale can obscure what matters to an owner. A pickup used mainly to carry passengers faces a different workload from one that regularly tows a heavy trailer; commercial and off-road work add still other demands. Compare engines in the vehicle and job you actually have in mind, rather than assuming a high position translates to the best choice for every truck.

- Match the application: Identify the truck class and whether the work is passenger transport, towing, commercial hauling, or off-road use.
- Match power and torque to the load: Requirements depend on the trailer, payload, terrain, and operating pattern.
- Compare fuel use under relevant conditions: Payload and drive cycle affect the result; unloaded or unrelated-cycle figures may not predict fuel use on your route.
- Consider ownership support: Maintenance complexity, parts access, service availability, and the vehicle’s model year matter alongside an engine’s reputation.
The U.S. Department of Transportation’s 2016 Commercial Medium- and Heavy-Duty Truck Fuel Efficiency Technology Study, Report 2 illustrates why conditions matter. In its modeled pickup comparisons across the listed cycles at 50% payload, the 2019 baseline 3.5 V-6 gasoline engine had an average fuel-consumption penalty of 4.1% relative to the 2019 diesel baseline; the baseline 6.2 V-8 had a 13.7% penalty. Those are study-specific modeled results, not universal real-world mileage figures or evidence about engine reliability. The report also notes diesel’s substantial efficiency advantage under extreme loads such as high-speed grades or heavy-trailer towing.

Use the engine family as a starting point, then verify information for the particular truck and model year. NHTSA’s official “Search Safety Issues” tools let you look up recalls and investigations. Those records can help identify documented safety issues and remedies, but they do not provide a standalone reliability ranking. NHTSA also distinguishes the dates shown for recalls from those shown for investigations, which refer to different events.

- Search NHTSA’s recall and investigation records for the vehicle and review the issue details, dates, and any stated remedy.
- For a purchase or repair decision, seek model-year-specific maintenance and repair information relevant to the truck’s actual duty cycle; the editorial ranking does not supply those records.
