Whiplash symptoms may take several hours to begin, and concussion symptoms can emerge immediately or hours or days afterward. A delayed symptom does not tell you what injury you have: seek emergency care for the warning signs below, and arrange medical advice for symptoms that persist or concern you.
Why do I feel fine after a car accident but hurt later?
Symptoms do not always appear at the scene. Adrenaline, distraction, and the nature of an injury may make it difficult to notice how you feel at first, but feeling well initially cannot rule out an injury. NHS guidance says whiplash symptoms can take several hours to start. CDC guidance says mild traumatic brain injury (TBI), including concussion, symptoms may appear immediately or hours or days later.
Pay attention to changes after the crash, but do not use a symptom list to diagnose yourself. Symptoms can overlap with other health problems, and a clinician can assess what is causing them.
Can pain start hours after a crash?
Whiplash and neck symptoms
Whiplash is commonly associated with traffic accidents. NHS guidance says symptoms may take several hours to begin and can include neck pain or stiffness, headache, and pain in the shoulders or arms. The NHS advises against using a neck brace or collar for whiplash.

Possible concussion symptoms
A concussion does not require a visible head wound. CDC guidance describes symptoms that can affect how someone feels, thinks, acts, or sleeps. Examples include headache, dizziness, nausea, balance or vision problems, tiredness, difficulty concentrating or remembering, irritability, sadness, and changes in sleep. Symptoms can be overlooked or mistaken for other problems.
When does a symptom need emergency care?
Possible concussion: emergency warning signs in CDC guidance
For an adult with a possible concussion, seek immediate emergency care if any of these signs occur. Do not wait to see whether they pass.

Neck injury: urgent warning signs in NHS guidance
The NHS advises urgent medical attention for severe neck pain, tingling, problems walking or sitting upright, an electric-shock sensation in the neck or back, or weakness in an arm or leg. This is UK NHS guidance; if you are elsewhere, use your local emergency service for urgent symptoms.
How long can pain and other effects last?
Recovery varies by injury and person. These timelines describe different conditions and are not a promise about an individual recovery.

These timeframes are not interchangeable: neck pain and concussion symptoms can have different courses, and more than one issue may be present.
How should I return to normal activities?
For mild TBI or concussion, CDC guidance describes a short initial period of rest followed by a gradual return to activity. Increase activity in stages and adjust if symptoms worsen. Ask a healthcare provider about returning to work, school, exercise, driving, and other activities in light of your symptoms and circumstances.

There is no universal date when driving is safe after a crash. CDC clinical guidance cautions that attention, processing speed, or reaction time affected by a concussion can make driving unsafe. Ask your clinician for advice if these abilities are affected; do not treat feeling generally better as automatic clearance to drive.

A crash can affect sleep, concentration, mood, work, relationships, and day-to-day responsibilities as well as the body. The World Health Organization’s post-crash response report says, “Survivors and families affected by road traffic crashes have a range of physical, psychological and legal needs.” It describes injury-related disability and psychological trauma as challenges that can disrupt reintegration into work and family life. The European Commission likewise describes post-impact care as including initial treatment and follow-up that can help address pain and distress, with rehabilitation and reintegration relevant after major injuries.

Emotional distress and PTSD
Distress after a frightening event does not by itself mean someone has PTSD. MedlinePlus says PTSD can develop after experiencing or witnessing trauma, including a car accident. Symptoms may involve re-experiencing the event, avoiding reminders, heightened arousal or reactivity, and changes in mood or thinking. They often begin soon after the event, but can sometimes appear months or years later. Only a qualified clinician can diagnose PTSD.
If emotional symptoms persist or interfere with sleep, relationships, work, or daily life, consider speaking with a healthcare professional. NHS guidance includes trauma-focused therapies such as trauma-focused cognitive behavioural therapy among treatment options for PTSD; that does not mean every crash survivor needs therapy.
What to do after a crash
- Seek emergency care for urgent warning signs. Use the relevant emergency guidance above rather than waiting for symptoms to settle.
- Arrange medical review for symptoms that concern you. This is especially important when symptoms persist, worsen, or affect normal activities; the NHS advises review if whiplash symptoms have not improved after one week.
- Track changes without trying to diagnose them. Note when symptoms began and how they affect sleep, concentration, movement, work, or daily tasks so you can describe them to a clinician.
- Return to activity gradually when advised. For possible concussion, follow a clinician’s advice on increasing activity and on driving, work, school, and exercise.
- Ask for support with practical and emotional effects. Recovery may involve family responsibilities, employment, rehabilitation, or psychological care in addition to medical treatment.
About statistics on longer-term effects
A historical NHTSA report, drawing on preliminary estimates from around 2000, estimated that at least 31,000 people had post-traumatic stress symptoms and at least 62,000 had major depressive symptoms one year after injury. NHTSA noted that the estimates were preliminary and that overlap between the groups was unknown. They are not current prevalence rates and should not be read as figures for all crash survivors.
