The NTSB’s completed investigation found that a 2021 Tesla Model 3 reached 90 mph while crossing a red-light intersection in a 30 mph zone in Coral Gables, Florida, and identified excessive speed as the probable cause of the fatal crash. The cited NTSB materials do not establish whether Full Self-Driving (FSD) was engaged or involved.
What happened in the Coral Gables crash?
At about 8:54 p.m. EDT on September 13, 2021, a 2021 Tesla Model 3 Long Range-Dual Motor was traveling north on Alhambra Circle in Coral Gables, southwest of downtown Miami. According to the NTSB’s completed investigation, the driver accelerated, moved into the opposing lane to pass another car, returned to the northbound lane, and continued accelerating. The Tesla ran a red light, left the road, and struck two trees in the center median. Both occupants died.
Investigators said the weather was clear, the roadway was dry, and streetlamps illuminated the area. They did not identify road conditions, weather, lighting, traffic signal timing, or signage as factors in the crash.
Was the Tesla Model 3 going 90 mph?
Yes. The NTSB reconstructed the vehicle’s speed using its event data recorder and video from a witness vehicle. In its 2023 account, the agency reported that the Tesla reached 90 mph while crossing the Coral Way intersection, where the posted speed limit was 30 mph. It was traveling about 81 mph immediately before striking the second tree; five seconds before that impact, it was traveling 68 mph. What did the NTSB conclude caused the crash?
In its final accident brief, dated February 13, 2023, the NTSB stated: “The National Transportation Safety Board determines that the probable cause of the Coral Gables, Florida, crash was the driver’s decision to travel at an excessive speed, which led to the failure of the driver to control his car.” NTSB final accident brief (HIR-23-03)
The report describes a possible additional loss-of-control factor without making it the primary cause. As the car crossed a change in roadway grade, its suspension became fully compressed and the undercarriage contacted the road. The electronic stability control system then entered a fault state. The NTSB said the loss of electronic stability control may have further contributed to the loss of control; its probable-cause determination, however, centered on the driver’s excessive speed. The report does not establish that a manufacturing defect caused the crash.

The NTSB also reported that the driver was actively engaged in the driving task, that records showed no phone calls or texts at the time, and that toxicology tests were negative for alcohol and the drugs tested. Those findings do not establish the driver’s intent or motivation.
Was FSD involved?
The cited NTSB case page and final accident brief do not state whether Full Self-Driving was engaged or involved. FSD involvement is therefore unestablished in these materials. They do not support saying that the system was off, that investigators ruled it out, or that Tesla’s system was cleared.

What did the report say about the fire and responders?
The passenger-side impact damaged the high-voltage battery and caused a postcrash fire. The NTSB reported that the car was engulfed within seconds and that responders managed reignition events. Responders told investigators they had not consulted an external source or emergency response guide at the scene.
The report references Tesla’s Model 3 Emergency Response Guide in the context of responder safety around electric-vehicle battery fires, damaged vehicles, and possible reignition. This is a detail about the response to this crash, not a recommendation for consumer fire equipment. NTSB final accident brief (HIR-23-03)

How do the preliminary reports differ from the final finding?
Early coverage, including an Associated Press report published November 11, 2021, described preliminary findings and recorded the family attorney’s dispute with the focus on driver behavior. The attorney, Aaron Davis, said: “You cannot market a car for its speed and its coolness and then blame the driver for doing the thing you are marketing.” That was an advocacy statement, not an NTSB finding.
