A Hyundai Accent rolled onto its roof near Marmion Avenue and Aviator Boulevard in Mindarie at about 4 a.m. on Saturday, 27 August 2022. Police initially reported four 18-year-old male occupants, one with life-threatening injuries and three with less serious injuries. Later reporting identified the critically injured occupant as Dominic Burns and said he died after his family decided to end life support following organ donation.
What happened in the Mindarie crash?
Western Australia Police said officers were called at about 4 a.m. to a crash near the intersection of Marmion Avenue and Aviator Boulevard. The vehicle was a Hyundai Accent; it had rolled and came to rest on its roof, with one occupant trapped. The police account was published by Mirage News on 27 August 2022.
Police said four 18-year-old males were believed to have been in the car. Their initial assessment was that one had life-threatening injuries and the other three had less serious injuries. PerthNow reported the same contemporaneous injury assessment, quoting an unnamed police spokesman: “One of the four males is believed to have life-threatening injuries, while the three other males have less serious injuries.”
What emergency services attended?
St John Ambulance and the Department of Fire and Emergency Services attended. An RAC Rescue helicopter was also deployed. The police release said one occupant was trapped in the vehicle; it did not provide further detail about the rescue or treatment at the scene.

What later reporting said about Dominic Burns
The initial police release described the injuries as they were understood when responders attended; it did not identify the occupants by name or report a later death. In subsequent coverage, The West Australian identified the critically injured occupant as 18-year-old Dominic Burns. The newspaper reported that his family decided to switch off life support after organ donation. That later information comes from The West Australian, not the initial police account.
What is known about the investigation?
Major Crash Investigation Section officers attended to investigate the cause. The police release did not announce a cause, final investigation findings or a legal outcome, and the available reporting does not establish what investigators ultimately concluded.

Police appeal for witnesses
Police asked anyone with information about the crash, dash-camera footage or mobile-phone vision to contact Crime Stoppers WA. The appeal was for people who might have relevant evidence about this incident.
