Include the date, location, vehicle make and model, company if known, and a factual description. For a federal vehicle-safety complaint, use NHTSA’s separate complaint form or call its hotline. A paid-ride or carrier-service dispute that remains unresolved belongs in the CPUC’s separate consumer process.
Choose the reporting channel that fits the problem
| What happened | Where to report it | Purpose |
|---|---|---|
| You observed a robotaxi incident or safety concern on a California public road | California DMV AV Incident Form | State channel for reporting an autonomous-vehicle road incident or concern. |
| You want to submit a federal vehicle-safety complaint | NHTSA Automated Vehicle Complaint Form or NHTSA’s Vehicle Safety Hotline, 1-888-327-4236 | Federal vehicle-safety complaint channel. The DMV also lists TTY service at 1-888-275-9171. |
| You have an unresolved complaint about a paid passenger ride or carrier service | CPUC Transportation Complaint process | Consumer process for passenger-carrier service disputes, after first contacting the company. |
These channels are separate. A DMV or NHTSA safety report does not automatically file a CPUC service complaint, or vice versa. You may use DMV and/or NHTSA for an observed safety event; use the CPUC process when seeking help with an unresolved passenger-carrier service dispute.

Submit a useful report to the DMV
- Open the official California DMV AV Incident Form.
- Enter the date of the incident and where you observed the vehicle.
- Provide the vehicle’s make and model and the company, if known. The form includes a license-plate field, but it is not marked as required.
- Describe what happened in plain, specific terms. Separate what you directly saw from what you infer; for example, describe the vehicle’s movement and the surrounding conditions rather than guessing why it acted that way.
- If a required detail is unknown, the form instructs you to enter “unk” or “unknown.”
- Do not include sensitive personal information such as a Social Security number, driver-license number, or financial-account number.
Contact details are optional, and the DMV says a reporter may remain anonymous. The form also cautions that without contact information it may be unable to reach you or conduct further investigation. Share contact details only if you are comfortable being contacted.

Starting June 26, 2026, this camera will include a 32GB memory card (previously 64GB) so you can still use it out of the box. For extended storage, we highly recommend purchasing a Class 10, U3, and V30 (or higher) memory card.

NHTSA is the federal vehicle-safety complaint route linked by the DMV. The CPUC handles passenger-carrier consumer complaints and publishes quarterly data about autonomous-vehicle passenger-service programs. Those quarterly reports are background records, not a way to submit an individual complaint; the CPUC says some material may be redacted while confidentiality claims are reviewed. Its quarterly-reporting page was last updated August 18, 2026.
The agencies’ published reporting pages do not promise a response time, investigation, or remedy for an individual submission. The DMV form is not an emergency service. If someone is in immediate danger, contact emergency services instead of relying on an online report.
