Check your vehicle identification number (VIN) in the NHTSA recall lookup or Honda/Acura’s recall lookup to find out whether your vehicle has an unrepaired recall. The 2021–2025 Acura TLX, 2023–2025 Acura MDX and 2023–2025 Honda Pilot are in the potentially affected population for recall 25V391, but a model-year match alone does not establish that an individual vehicle is included. The prescribed dealer inspection and any necessary repair are free.
Which Honda and Acura vehicles are covered?
NHTSA campaign 25V391 covers certain—not all—vehicles in these model-year ranges:
- 2021–2025 Acura TLX
- 2023–2025 Acura MDX
- 2023–2025 Honda Pilot
NHTSA’s June 2025 acknowledgement lists 259,033 potentially affected vehicles. American Honda Motor Co. described the U.S. recall as covering about 259,000 vehicles in its June 18, 2025 announcement. The precise NHTSA figure is the estimated population, not a count of vehicles confirmed to have the defect.
To determine whether a specific vehicle has an open recall, search its VIN through the NHTSA recall lookup or Honda/Acura’s recall lookup. NHTSA explains that searches by year, make and model provide general recall information; a VIN or license-plate search is intended to show unrepaired recalls for the specific vehicle. A newly announced recall may not yet have every VIN identified, so check again if a result is inconclusive.

What is wrong with the brake pedal?
A pivot pin in the brake pedal assembly may not have been staked—the securing step that keeps it in place. If the pin is not secured correctly, the brake pedal can shift laterally or move out of position. NHTSA’s recall report identifies the affected components as hydraulic service brake pedals and linkages and says the vehicles fail to comply with FMVSS 108 and FMVSS 135.
How the installation defect happened
According to Honda’s report to NHTSA, supplier Otsuka Koki Co., Ltd. was transitioning production from a U.S. plant to a plant in Mexico and increased staffing to build inventory. Insufficient training led to missed staking steps. A QR-code scanning system intended to prevent omitted steps malfunctioned; the supplier disabled it and continued production, allowing the issue to go undetected.

The report says the improper staking occurred at the supplier’s U.S. plant, which later closed. Around November 6, 2024, the Mexico plant began using a camera sensor to verify that the pivot pin had been staked. NHTSA’s report says remedy parts have properly staked pins.
What symptoms should drivers watch for?
Honda’s recall materials list abnormal brake-pedal movement or feel, a brake malfunction warning light, and brake lights that remain illuminated when the pedal is not pressed. The defect may also cause unintended brake application or make it difficult to apply the brakes as intended, increasing the risk of a crash or injury.

Symptoms are not a reliable way to determine whether a vehicle is included: check the VIN even if the pedal feels normal. If the pedal behaves abnormally or you cannot safely slow or stop, do not continue driving it; arrange professional assistance.
What should an owner do?
- Look up the VIN. Use the NHTSA or Honda/Acura recall lookup to check for an unrepaired recall on your specific vehicle.
- Contact an authorized Honda or Acura dealer. Arrange an appointment if the VIN shows an open recall. Honda’s June 18, 2025 announcement said replacement parts were available.
- Have the dealer inspect the assembly. The dealer will replace the brake pedal assembly if needed, at no cost to the owner. This is the official remedy; do not attempt an owner-directed adjustment or buy parts as a substitute.
If you already paid for a repair related to this issue, NHTSA’s report says you may be eligible for reimbursement under the recall reimbursement plan on file. Confirm eligibility with Honda or Acura rather than assuming the expense will be reimbursed.

What do the recall dates and reported claims mean?
NHTSA’s campaign report is dated June 12, 2025; its acknowledgement is dated June 17, and Honda announced the recall on June 18, 2025. The report says dealer notification was scheduled for June 13, 2025, and owner notification for July 28, 2025. Those are dates from the original documents, not confirmation that a particular vehicle has since been repaired.
In its report, Honda recorded three warranty claims and no injury or death reports through June 5, 2025. Honda’s June 18 public release separately said it had received no confirmed reports of crashes or injuries at that time. These dated figures describe what had been reported then; they do not establish the current condition of an individual vehicle.
