No injuries were reported, and both vehicles sustained minor damage. Zoox said its automated-driving system incorrectly predicted what the other vehicle would do after it approached from the side and stopped.
The recall, announced May 6, 2025, covered 270 vehicles or automated-driving systems running software released before April 17. Zoox had already deployed corrective software to affected vehicles operating on public roads, but the company still reported the defect to the National Highway Traffic Safety Administration (NHTSA) under a formal recall campaign.

What happened in the Las Vegas crash?
The Zoox vehicle was unoccupied and operating autonomously in Las Vegas. A passenger car approached the roadway from a commercial driveway or another perpendicular direction.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsAccording to Zoox’s account, the system predicted that the passenger car would continue moving. The car instead stopped and yielded in the shoulder area. The Zoox robotaxi slowed and steered right, then braked hard when the situation developed, but it could not avoid contact.

Neither vehicle was carrying a reported passenger in the collision. No injuries were reported, and the damage to both vehicles was minor. Zoox notified the Las Vegas Metropolitan Police Department through its remote-operations team.
That sequence matters. The available recall documents do not describe a moving car unexpectedly cutting off the robotaxi. They describe a prediction error: the other driver stopped and yielded, while the automated-driving system made an inaccurately confident assessment of the vehicle’s likely behavior. Zoox identified that software behavior as the safety defect; the documents do not establish a legal finding of civil or criminal fault.
Zoox’s incident and recall report provides its account of the collision, while NHTSA’s Part 573 filing records the formal defect description and response timeline.
What did Zoox recall?
This was principally a software recall, not a conventional mechanical repair involving a privately owned car.
- NHTSA campaign: 25E-029
- Zoox recall number: 25-02
- Affected population: 270 vehicles or automated-driving-system-equipped units
- Affected software: ADS versions released before April 17, 2025
- Remedy: Updated ADS software, provided at no charge
NHTSA described the relevant condition as an inaccurate prediction when another vehicle approached slowly from a perpendicular direction and stopped. At speeds above 40 mph, the system could lack enough time or path options to avoid a collision.

That does not mean the system necessarily failed to detect the other car. The evidence supports a more specific conclusion about behavior prediction and collision avoidance, not a general sensor-visibility failure.
Software recalls can look different from recalls involving brakes, airbags or steering components. Zoox owns and operates its robotaxi fleet, so it can deploy a correction across fleet vehicles instead of sending individual consumers to dealerships. In this case, Zoox had already installed the updated software on affected vehicles operating on public roads before the recall announcement. NHTSA’s acknowledgment letter identifies the campaign, affected software and remedy.
Why was there a recall if the software had already been updated?
A recall is a formal safety-reporting and remedy process, not necessarily an announcement that the defective software is still operating everywhere. Zoox’s internal investigation identified the problem, developed and tested an update, and deployed it between April 16 and April 17, 2025. The company then filed its voluntary Part 573 recall report.
The formal filing still mattered because it documented the affected software population, the safety risk and the remedy for regulators. It also created a record of completion. In NHTSA’s quarterly report submitted July 29, 2025, all 270 affected units were listed as remedied, with zero unreachable and zero removed.
The 270 figure should not be read as Zoox’s entire fleet. It represented the vehicles or automated-driving systems equipped with the relevant software, not necessarily every vehicle the company owned or operated.

Similarly, the update addressed the identified prediction condition. It does not establish that Zoox’s technology—or autonomous driving generally—is free of every other possible failure mode.

Did Zoox stop operating after the crash?
Zoox paused driverless operations across its operating geofences immediately after the incident while it conducted an internal safety review. That was a pause in driverless operations, not a permanent shutdown of the company.
Operations resumed in lower-speed geofences on April 10. Las Vegas routes above 40 mph remained excluded during part of the investigation. After Zoox completed testing and deployed the updated software, operations resumed across all geofences, according to the NHTSA filing.
The response included:
- Notifying local law enforcement through remote operations.
- Pausing driverless service and reviewing the incident.
- Reporting the event to NHTSA under the agency’s Standing General Order.
- Analyzing the behavior and reproducing the scenario through simulation.
- Developing, testing and validating a software correction.
- Deploying the update to affected vehicles.
- Filing a voluntary formal safety recall.
Was this Zoox’s first recall?
No. Zoox had already filed a separate voluntary software recall in March 2025. That recall involved previous software associated with two incidents in which retrofitted Toyota Highlander test vehicles were rear-ended by motorcycles after braking. Those vehicles had safety drivers, and Zoox said the affected software was no longer in use.
The March incidents involved a different vehicle type, operating configuration and failure pattern from the April Las Vegas collision. Zoox’s March safety-recall report describes that earlier action.

What was the separate San Francisco e-scooter incident?
Zoox reported another, separate incident in San Francisco on May 8, 2025. An unoccupied robotaxi braked at an intersection to yield, and an e-scooter rider struck the vehicle. Zoox said the rider suffered minor injuries, declined medical attention and that the vehicle was operating as expected when it was hit.
Even so, Zoox filed another voluntary software recall to improve perception tracking and reduce the chance of the vehicle moving when a vulnerable road user is very close.
This event should not be merged with the Las Vegas crash. The dates, locations, road users and software concerns were different. Zoox’s San Francisco recall report covers that incident.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Did the recall cancel Zoox’s Las Vegas launch?
No. Contemporary reporting said Zoox still planned to begin commercial service in Las Vegas later in 2025. The company subsequently announced that public Las Vegas service launched on September 10, 2025. Zoox’s Las Vegas service page records the launch.

The launch does not make the April crash insignificant, nor does it prove that all autonomous-driving risks had been solved. It shows that the recall and operational pause did not permanently cancel the Las Vegas program.

As of 2026, Zoox has continued expanding its service and testing footprint. That later activity is useful context, but it should not be treated as evidence that the recalled software was harmless. The relevant conclusion is narrower: Zoox identified a specific software condition, corrected the affected public-road vehicles and resumed operations after review and validation.
How serious was the incident?
Its immediate consequences were limited: no reported injuries and minor damage. But calling it merely a harmless fender-bender would miss why the event triggered a formal recall.
The incident exposed a repeatable class of risk involving an approaching vehicle, an unexpected stop and an overly confident automated prediction. The issue was serious enough for Zoox to pause driverless operations, restrict higher-speed routes during the investigation, develop a software fix and notify NHTSA.
The most accurate characterization is therefore mixed:
- Immediate severity: Low in this particular collision.
- Safety significance: Material, because the defect could increase crash risk in similar situations.
- Operational significance: High enough to prompt a company-wide pause in driverless operations across its geofences.
- Regulatory significance: Substantial enough to require a formal safety-recall filing.
What this recall says about autonomous-vehicle reliability
The episode illustrates an important distinction in autonomous driving. Seeing another vehicle is not the same as correctly predicting its next action. A system may detect an object, classify it as a vehicle and track its position, yet still make the wrong forecast about whether the vehicle will continue, stop or yield.
It also shows why software updates and recall reporting remain important even for fleet-operated autonomous vehicles. The operator can deploy a correction quickly, but the underlying defect may still require formal documentation, operational restrictions and confirmation that the affected population has been remedied.
At the same time, one collision cannot establish that all Zoox robotaxis are broadly unsafe. The recall covered a defined set of software versions and a particular interaction at speeds above 40 mph. Nor does the absence of injuries prove that the software was acceptable. A low-consequence outcome can coexist with a meaningful systemic safety problem.
