Three boys in an SUV were reported unrestrained after a crash with a cement mixer killed the SUV’s driver and injured four other people on Oct. 5, 2026, in Olive Township, Ottawa County, Michigan. Preliminary findings attributed to the Ottawa County Sheriff’s Office say the westbound SUV entered the path of the northbound mixer at 120th Avenue and Stanton Street.
What happened in the Ottawa County crash?
The collision occurred at 120th Avenue and Stanton Street in Olive Township on Monday, Oct. 5. Local reports published the next day placed the crash between about 2:20 and 2:35 p.m.; the reports differ by several minutes, so the date and intersection are the clearest confirmed details. WGHN and Ottawa News Network reported that the mixer was heading north on 120th Avenue when the westbound Toyota SUV entered its path.
Preliminary findings attributed to sheriff’s investigators say the SUV failed to yield. That is an investigative account reported on Oct. 6, not a final legal determination of fault. The vehicles were identified in local coverage as a 2024 Toyota SUV and a 2016 Phoenix cement mixer. The Holland Sentinel also reported the crash and preliminary account.
How many people were killed or injured?
The 51-year-old SUV driver died at the scene. The 46-year-old front passenger was taken to Corewell Health in critical condition. Three boys, ages 4, 9 and 11, were taken to Helen DeVos Children’s Hospital: two were reported critically injured and one moderately injured. The cement-truck driver was reported uninjured.

Were the children wearing seat belts?
Local reports citing the sheriff’s office said all three boys were unrestrained. The two adults in the SUV were reported to have been wearing seat belts. The coverage does not provide the children’s heights, weights, seating positions or restraint-fit details, and it does not establish that their restraint status caused any particular injury.
What is known about the investigation?
The Oct. 6 reports said the crash remained under investigation and that the occupants’ identities had not been released pending family notification.
