What happened at the Rivian warehouse
TechCrunch reported on March 6, 2026, that contractor Kevin Lancaster, 61, died after an incident at a Rivian warehouse in Illinois earlier that week. The outlet attributed the account to local reporting and said Lancaster was pinned between a tractor-trailer and a loading dock. It reported that emergency crews were called at 1:40 p.m. local time and that he was pronounced dead at a local medical center at 2:33 p.m.

TechCrunch relayed that local authorities identified Lancaster and gave his cause of death as “blunt traumatic compressional injuries.” That description is attributed to local authorities in the news report; it is not an OSHA conclusion.
What OSHA’s public record shows
OSHA’s establishment search lists a Rivian inspection in Illinois opened March 5, 2026, with activity number 1879095.015. The record labels the inspection “Unprog Rel” and “Partial.” Its employer, state and opening date align with TechCrunch’s account, but the listing itself does not describe the fatality. OSHA cautions that open inspection information is dynamic and may be corrected, so this record is not a final determination.

Who is investigating and what remains unknown
At the time of TechCrunch’s report, the Normal Police Department and McLean County Coroner were also investigating. TechCrunch reported that OSHA told the outlet its probe could take up to six months. Rivian told TechCrunch: “Safety at our facilities is our top priority. Unfortunately yesterday afternoon, a contractor passed away after an incident at our warehouse. Our sympathy and thoughts are with their family and friends. We are working with the Normal Police Department on its investigation.”
The available sources do not establish OSHA’s ultimate findings or whether the agency will issue citations. OSHA explains that investigation summaries generally describe events and causal factors, but summaries for investigations completed within the previous year may not yet be available while the agency finishes its work and screens records. Why an OSHA data page may not show the full case
OSHA’s Fatality Inspection Data page is updated daily, but its display includes only inspections with an opening-conference date within the preceding six months. OSHA also says a victim’s name may be absent for jurisdictional reasons. Because the display is limited and inspection records can change, a missing entry there would not, by itself, show that no investigation occurred or that a case is closed.
